Thanks for completing the two questionnaires, we’ve attached them to your file to be reviewed by your practitioner.
In the meantime we’ve put together some things to know before your appointment:
Do you have recent test results?
To gain the most out of your initial appointment, please forward a copy of any recent pathology or test results to: info@mullenhealth.com.au or bring them with you on the day of your appointment and we can scan them in for you.

How to find us
We’re often asked about parking here in Hamilton – please note; 2 hour (non-metered) on-street parking is available around the building. At times it can get busy, be mindful it may take a few minutes to find a suitable parking spot. If you cannot manage to get a park, you’re more than welcome to park in the driveway.
Please note Murray Street is a one-way street. Access into Murray Street is a left hand turn off Tudor Street, please see the map below. We recommend using your GPS to find us.

Cancellation & Rescheduling Policy
A cancellation/rescheduling of your appointment within 24 hours of the scheduled time will result in a $50 fee or the forfeit of your deposit.
With little notice it is a financial loss to the business and a missed opportunity for another client (historically there is a 3-4 week waitlist). While there is a priority list, few people are ever able to replace your appointment with 24 hours notice.
We are more than happy to change your appointment to a telehealth consultation to avoid this fee.
* Please note, if you leave a message with the clinic over the weekend or after hours, will also result in your $50 booking fee being forfeited as we’re not here to try and fill your appointment.
Appointment Confirmation
Your appointment is not confirmed until you reply to our confirmation texts, emails or by calling our clinic and verbally confirming your appointment.
If your booking is still unconfirmed within 24 hours and we have made all efforts to reach you and have been unsuccessful in contacting you, we reserve the right to cancel your appointment.
Failure to Attend Appointment
Failing to attend an appointment will result in a $50 fee or the loss of your initial booking fee. Recurrence of not attending your appointments without 24hr notice will result in a booking fee of $50 in advance before you can make a future appointment.
